Area physicians who have placed patients in our care have been very pleased; not only with how we treat patients, but also with how we treat physicians.

In an effort to simplify and speed up the referral process, we have implemented a new fax referral procedure. There are three referral forms—one each for Medical Oncology, Radiation Oncology and Gynecologic Oncology—which you’ll want to make copies of for future use.

Please fill out the appropriate form, attach all requested documentation and fax to the number at the top of the form. Once we receive it, we’ll schedule the appointment, then fax the referral form back to you with the appointment date and time along wit h the name of the physician who will see the patient.

We will notify you within 2 business days of the appointment date and time—though we will make every effort to do so the same day. In addition, we would prefer that you contact the patient with the appointment information. We, in turn, will send the patient a New Patient Information Packet with details of the appointment and a map with directions to our facility.
