New Survivorship Clinic!
Life after a cancer diagnosis can sometimes present physical and emotional challenges. It’s not unusual for cancer survivors to complete all or the majority of their treatment and have concerns about lingering side effects or other healthcare issues. To make sure that our patients’ concerns are addressed, the West Michigan Cancer Center has opened a Survivorship Clinic.
The WMCC Survivorship Clinic is a customized, follow-up program for cancer survivors. Patients in the program receive careful monitoring for a possible return of their cancer, as well as help with side effects and other issues. We’ve assembled a team of experts in cancer care and survivorship to address our patients’ unique needs. Common issues following treatment can include pain, difficulty coping, fatigue and sleep problems, family issues, sexual side effects, financial woes and emotional adjustment, among other complications.
At the first Survivorship Clinic visit, a mid-level provider—usually a physician assistant or nurse practitioner—will meet with the patient to assess his or her specific needs and develop a plan of care. This appointment is billed through the patient’s insurance company as a normal visit to WMCC known as an ‘office visit’. Services provided by the Survivorship Clinic’s social workers, dietitians and financial counselors are offered at no charge.
For more information on the benefits of our new Survivorship Clinic, just click on the brochure. Patients are encouraged to review this option with their doctor as part of their ongoing cancer care. This program is in addition to regular visits, includes the patient’s cancer doctor in communication as well as recommendations for care to stay as healthy as possible, as long as possible. Before Your Visit
Before your visit to the Survivorship Clinic, please complete this Quality of Life Evaluation. The information you provide will help us continue to provide care specific to your needs. This form will be reviewed by the Survivorship Clinical Team to help them provide you with recommendations, and resources for cancer survivorship journey. Information you share will become part of your medical record, and is protected by HIPAA.
